Electronic forms:
Medical Release – Use this form if you are requesting Derry Imaging to send or obtain (images/reports) from another facility/provider.
CD/REPORT REQUEST – Use this form if you are requesting a copy of your images/report for personal use or to hand deliver to your doctor. Your request will be ready in 48 hrs unless it is for mammography, which can take up to 72hrs. You may also access your imaging and reports via our Patient Portal
